Cypress Pointe Surgical Hospital case management involves the entire medical interdisciplinary medical team working together to provide the best quality healthcare through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Case Management is accountable for the care, coordination and discharge planning of all patients. In addition to being advocates for patients, case managers work with their physicians and insurance companies to coordinate needed services.

Our case managers are trained professional nurses that can assist you by:

  • Evaluating options for your continuum of care with our extended medical staff. For example: long term treatment needs.
  • Assisting with arrangements for care after discharge from the hospital by coordinating the integration of social services/case management functions into patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies an healthcare facilities.
  • Making referrals to community resources
  • Determining what insurance benefits you have and match your needs to your benefits
  • Patient advocate between facility and insurance company to assure appropriate discharge plan.
  • Conducting reviews for appropriate utilization of services from admission through discharge.
  • Coordinating the provision of social services to patients, families, and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from healthcare services.
